Preventing ACL injuries


Did you know that women are up to 8 TIMES more likely for tearing their ACL, and 2/3rd of these injuries are non-contact, with the majority occurring from jumping and landing?  Several structural factors may put women at a higher risk of injury including smaller intercondylar femoral notches, increased Q angle, greater ligamentous laxity,as well as hormonal factors.  Although each of these factors may play a role, no evidence of any kind can accurately demonstrate a difference in ACL injuries involving any of these aforementioned factors.  Further, interventions aimed at injury reduction are unlikely to influence anatomic and hormonal factors.  Therefore, all efforts should be focused on improving neuromuscular control; specifically, having correct alignment throughout the movement, producing optimal muscle recruitment, and ensuring accurate firing patterns of the muscles that control the knee during movement.
During jumping, women demonstrate a greater hip adduction and internal rotation moment, simply meaning their knee collapses in (also known as "valgus collapse").  One research study demonstrates that just a slight valgus collapse will increase the load on the ACL by 9-fold.  Poor neuromuscular control at the hip and ankle may contribute to the valgus collapse and subsequent ACL ruptures. Another scientific study discovered that women have different neuromuscular control  patterns than men and do not have good control or facilitation over the correct muscles needed to stabilize the knee and prevent ACL injuries.

At Direct Performance PT, you will receive an individualized assessment that finds those pesky neuromuscular deficits and then we will immediately begin correcting the movements with proven interventions meant to resolve those deficits for good.  Exercises will isolate, stregthen, and gain neuromuscular control of the correct muscles that stabilize the knee, hip, and ankle during dynamic movements and sporting events.
